	COMPLETE THE FORMS ON LINE : Go to http://per.lacity.org/psb/onlineapp/ to complete the PBA and JPQ online and print.
	DOWNLOAD THE PHF FORM: If you make an appointment the Personal History Packet will be mailed to you. But if you want to get a head start click here to download the Personal History Form (PHF) pdf file. Complete the form and take it with you to the testing location.
