Forms for Submission and/or Signature when Contracting with LAWA

The following forms may be requested or required when doing business with Los Angeles World Airports. Please check your Request for Bid or Request for Proposal for complete details on all forms required.

Forms and explanatory documents for each of the following administrative requirements are identified below. Also included is a checklist to assist with proper completion of the required forms prior to bid/proposal submittal. The administrative requirements may reference the Los Angeles City Charter (LACC), Los Angeles Municipal Code (LAMC), and Los Angeles Administrative Code (LAAC). Copies of these documents may be available on the Internet at http://www.cityofla.org/lacity102.htm.

  • Forms that must be read and agreed to but do not require signed copy for submission:

  • Airport Concessions Disadvantaged Business Enterprises Program

  • Municipal Lobbying Ordinance

  • Bid Bond

  • Living Wage and Service Worker Retention Ordinance

  • Contractor Responsibility Program
  • Disadvantaged Business Enterprise Program

  • Minority, Women and Other Business Enterprise Program
  • Items required for all awarded contractors - to be obtained after bid has been awarded:

  • First Source Hiring Program

  • Insurance

  • Business Tax Registration Certificate

  • Equal Benefits Ordinance
  • Other Items to Note:
